How do I change the beneficiary of my 529 Education Savings Account?

Here are the steps to change your beneficiary:

Print the attached Form 510 – Beneficiary Change
Enter the new beneficiary’s information, including full name, birth date, and Social Security Number.
Sign and date Section 5
Mail the completed Form 510 to:

Salt Lake City, UT 84114-5100
